For residents across Sunriver and the surrounding South County area, access to compassionate, well-coordinated care can make all the difference, especially during life’s most vulnerable moments. Since 2011, Molly Denton, a transitions coordinator and certified Community Health Worker (CHW) with Partners In Care, has played a vital role in helping individuals and families navigate that journey.
Working throughout Central Oregon, Denton has witnessed firsthand how much the region’s health and support systems have evolved. With that growth, however, comes complexity. “Our role often becomes the missing piece,” she explains, helping bridge gaps, connect resources, and ensure that no one feels lost in the system. At the heart of her work is a simple philosophy: getting to truly know each person. By understanding individual needs, values, and circumstances, she helps align services that enhance quality of life and support people in continuing to live fully, even through significant change.
Denton’s service area stretches far beyond Sunriver, covering south Bend to La Pine and reaching into rural communities across Klamath and Lake Counties. While resources can be more limited outside Deschutes County, she works diligently to close those gaps. Through close collaboration with Aging & People with Disabilities offices and Council on Aging, Denton helps ensure residents, no matter how remote, have access to meaningful support.
A key part of her work at Partners In Care comes through the Transitions team, which supports families facing major health changes for no cost. The focus is not only on clinical coordination, but on emotional support as well. “We aim to be a steady presence,” Denton says, offering reassurance to both patients and caregivers as needs evolve.
That commitment extends beyond one-on-one care. Denton also facilitates Partners In Care’s Caregiver Café, a welcoming space where caregivers can connect, share challenges, and celebrate small victories together. In a region defined by its strong sense of community, her work reflects a deeper truth: no one should have to navigate life’s toughest moments alone.
